Please note that requests for image prints from MOHAI’s photo collection are temporarily on hold through May 2012 while the collection moves to the new MOHAI Resource Center. Please check back in June 2012.

I understand that my usage of photographs will be limited as per both the general usage chosen above and the intended usages specified below. All other rights are reserved to the Museum of History & Industry.

Examples are student classroom use, books, magazines, posters, internet, broadcast television etc. - these are just examples, please be specific about your intended use

This form is a worksheet only. We will contact you with a final estimate for your confirmation before beginning work on the order. For commercial use there will be additional fees to be confirmed, see the price list.
